FederalSame rules in every state
See what you pay for Medicare Part B and Part D in 2026, and whether your income adds a surcharge (IRMAA).
Medicare is federal and the same in every state. 2026 premiums use the income on your 2024 tax return. 2027 amounts come out in November.

Married filing separately and lived with your spouse: $202.90 under $109,000, $649.20 from $109,000 to $391,000, and $689.90 above that. The Part B deductible for 2026 is $283.
An extra charge on Medicare Part B and Part D premiums for people with higher income. Social Security looks at your tax return from two years ago, so 2026 premiums use 2024 income.
If it dropped because of retirement, marriage, divorce, a spouse's death or another life-changing event, ask Social Security to use your newer income with Form SSA-44.
IRMAA tiers are cliffs. Going $1 over a limit adds the whole surcharge for the year. If you are close, timing a Roth conversion or a large withdrawal can matter.
If you get Social Security, Part B comes out of your check before it is deposited. Otherwise Medicare sends a bill.
Yes. If your income and savings are low, a Medicare Savings Program can pay your Part B premium. Oregon and Washington run these through OHP and Apple Health. See the benefits calculator.
Medicare usually announces next year's Part B premium in November. We'll update this page when it does. 2027 premiums will use 2025 income.
